At the completion of the spell, and once per round thereafter, the target must succeed on a Fortitude save or be aged by one age category (from young adult to middle aged, from middle aged to old, and from old to venerable). Multiple failed saves result in further aging. Succeeding on two consecutive Fortitude saves ends the spell.
The target suffers the appropriate physical ability score penalties for aging, but does not gain any mental ability score bonuses. A venerable (naturally or not) target that fails its Fortitude save instantly dies. This is not a true death by old age, and the subject can thus be raised normally.
The age category advancement is instantaneous in nature, it is not reversed when the spell ends. A spell that breaks a curse (see: Removing a Curse) can restore the creature’s true age category.
